Building Regulation 2021 commenced on 1 September 2021 and replaces the Building Regulation 2006. Significant changes have been made to the Queensland Building Regulation forms: Read the Building Regulation 2021

Below is an explanation each certificate / forms and when they will be issued by Architectural Windows & Doors Pty Ltd

Certificates and Compliance Forms

1. Form 15 - Compliance certificate for specification (QLD)
e.g. Supply Only Products

The appointed competent person (design – specifications) completes the Form 15, which is used to inform the building certifier’s decision when they are assessing a building development application and issuing the building development approval for the building work the subject of this form. A new appendix provides information for the appointed competent person, the building certifier, building product manufacturers and suppliers generally about the form.
A manufacturer or supplier of building materials may give evidence of suitability such as a manufacturers statement for an aspect or material that it is compliant with to indicate its compliance with the relevant reference documents in the Building Code of Australia i.e. the applicable Australian Standard/s.
A Form 15 will be issued upon completion of the supply according to the confirmed quotation and providing there are no payment claims outstanding.

2. Form 43 - Aspect Certificate (QLD)
e.g. Installation of Products

Form 43 is to be completed by a QBCC licensee carrying out the aspect work that is subject to a BDA for a single detached class 1a building or a class 10 building or structure.
A Form 43 can be issued by either a licensed person or a licensed corporation and is given to the building certifier. The QBCC licensee must complete the form informing the building certifier how the aspect work complies with the BDA. 
Where the "Installation of ..."  is specifically listed on the confirmed quotation (this excludes on-site screen fitting / servicing), a Form 43 will be provided by our Company will list which of items supplied have been installed into the building's structure (prepared by others) and the aspect of the work with reference to the code compliance. 
A Form 43 is issued upon completion of the project and providing there are no payment claims outstanding.

3. Window and Door Compliance Certificate (NSW)
e.g. Supply Only Products

Our Compliance Certificate is based on the Australian Glass and Window Association Accredited Member template and is issued upon completion of supply and after all finalised payments have been received.

The certificate will list specified products or reference the confirmed quotation for the schedule of items supplied to your project and will state the compliance with the relevant Australian Standards and other such product performance information.
This is the only form we issue to orders supplied to projects in New South Wales
For orders supplied in Queensland, the same information which appears on the compliance certificate is replaced by the Form 15; separate compliance certificate will only be issued by special request from the building certifier.

Forms Not Issued 

Form 12

Form 12 is to be completed by an appointed competent person after they have inspected an aspect of work and are satisfied the aspect has been completed and complies with the BDA. The form is given to the building certifier to assist them to finalise the stage of the building work. For example, it may be used by an electrician or engineer (whom the building certifier has deemed competent) for an aspect of the building stage relevant to their experience, skills, and qualifications (and licence/registration if applicable). Nb: A Form 12 cannot be issued by a licensed corporation.

Form 16
 

The Form 16 is an ‘Inspection Certificate’ and is to be used to sign off on a stage of work for all building classes. It is to be completed by the relevant building certifier, another building certifier or an appointed competent person (approved by the building certifier) when inspecting a stage of the work for compliance with the BDA.
Each stage of the building work must be inspected and only the building certifier may complete a Form 16 for the footing stage, slab stage and final stage of the work.

Nb: If an installation service is provided for the items supplied (into the building's structure which are prepared by others): a Form 43 will be provided for this Aspect of the work with reference to the code compliance. Refer to the Form 43 Section
